MANDATORY: set up direct deposit of your paycheck through Self-Service website at: https://login.uiowa.edu/uip/login.page?service=https://hris.uiowa.edu/portal/
- NO LATER THAN August 24, 2012 or else a paper check will be created and sent to the Payroll Office in 120 USB and you will be required to pick it up in person and set up direct deposit at that time.
MANDATORY: complete W-4 tax withholding form through Self-Service website at: https://login.uiowa.edu/uip/login.page?service=https://hris.uiowa.edu/portal/
- NO LATER THAN August 24, 2012 or else the University is required to withhold at the highest rate possible for federal and state income tax
MANDATORY for Health Science and International Students (Optional for others): complete employed graduate student insurance application to enroll in health and/or dental insurance benefits at: http://www.uiowa.edu/hr/benefits/health/student/empgradapp.pdf
- NO LATER THAN: September 7, 2012
